Hospice Registered Nurse / RN Case Manager / Part time

Job Description
Interim HealthCare Hospice of Southeast Ohio is expanding and looking it add a RN Case Manager to the team.? ?This is a Part Time position with the potential for full time in the future.
Duties of RN Case Manager in Hospice? include but are not limited to:

  • Plans, organizes, directs and provides hospice care to meet the identified needs of patients/families in accordance with Interim HealthCare Hospice policies and procedures, accepted standards of nursing practice and palliative care, and applicable law and regulation

  • Managing the care of defined caseload to facilitate the healthcare team's efficient and effective achievement of patient outcomes in accordance with the patient's plan of care.

  • Providing patient care and coordinating the services of a multidisciplinary team in collaboration with the patient's physician.

  • Completing initial and on-going comprehensive assessments of the patient's needs and requests at appropriate time points assigned. Initiating the plan of care and revising as necessary.

  • Providing those services and/or treatments requiring substantial and specialized nursing skills.

Job Requirements

  • Graduate of an accredited school of nursing.

  • Licensed to practice as a RN in the state of Ohio. ?

  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.?

  • If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program

  • One (1) year of hospice care experience preferred.

  • Valid driver?s license and?auto liability insurance?

??????????? Able to comply with accepted professional standards of nursing practice and law and regulation.
??????????? Understands principles of pain/symptom management.
??????????? Able to read and interpret technical instructions related to the care of the patient.
??????????? Knowledge and commitment to the hospice philosophy of care.
??????????? Demonstrates excellent observation, verbal and written communication skills, and problem solving skills, basic math skills; nursing skills per competency checklist.
